I've recently seen a problem with my qmail-ldap installation. I think it
appeared either after I upgraded to openldap v2.0.23, or when I started
adding indexes to slapd.conf. Here's a log extract;

Any clues to where I should start to look? My indexes look like this;

index   objectClass                                     eq
index   mail                                            pres,sub,eq
index   uid,accountStatus,userPassword                  pres,eq
index   mailMessageStore,mailAlternateAddress           pres,eq
index   mailForwardingAddress                           pres,eq
