I actually just did an article on this very thing:

(Note, this is for a public address book only.)

That should provide you with the details to get rolling. You can easily expand this to integrate with the way that qmail-ldap works for both an address book and authentication system.

input from qmail-ldap admins who have done this, so it integrates
