Please try to chcek ur start up script of POP3 and SMTP in /var/qmail/bin/pop3d/run 
and /var/qmail/bin/smtpd/run
 
Put parameters -R ot -H

                I have a qmail-ldap installation with 15k+ users on the system, the
        specs as follows:
        
        quad Xeon 550 MHz with 1 MB cache
        1 GB RAM
        /var is mounted on a Mylex DAC1164P with 4 Ultra SCSI 3 HDD in RAID 5
        OpenLDAP on another Dual processor 933 MHz server.
        System is running RedHat 7.1 with latest kernel from RedHat
        
        The problem is that once we get a dictionary attack on the server the
        'not yet preprocessed' gets bigger and bigger while checking on
        qmail-send the deliveries are so little
        
        the local deliveries are set to 500 and the incoming (smtpd) is set to
        300, where is the problem.
