do this
ps -wwef | grep proctitle, this (on Linux) should show if any error are there. Also check permission and ownerships on


/var/log/qmail/

This is your smtpd logs which shows that another copy of qmail-smtpd is running (maybe /service/qmail-smtpd/run is not correct), this has nothing to do with the delivery, for the delivery take a look at the qmail-send log
