Hi James,

I had also a problem with tls. It seams to be that some outlook versions
have problems with tls. For outlook express update via:
windowsupdate.microsoft.com and for the office version of outlook via
officeupdate.microsoft.com. You can also search for a single tls patch.
Unfortunatly I do not know the knowledge base number.

Can you have a look in the qmail-smtpd's logfile?

( /var/qmail/log/qmail-smtpd/current )

And increase the LOGLEVEL (here: 3) in control/qmail-smtpd.rules 
After that, don't forget to run 'make'
