You could follow qmail installation from: http://www.lifewithqmail.org, http://www.lifewithqmail.org/ldap, and http://www.qmail-ldap.org/index.php?title=INSTALL
mixed them together using stock qmail tarball from: http://cr.yp.to/qmail.html and patch it with the latest qmail-ldap patch from: http://www.qmail-ldap.org then configure the run-script and controls files. So much painfull for me. For migrating mysql to LDAP I think you should create script to convert to ldif format and upload it to LDAP server.
