we're using qmail-ldap on OpenBSD, together with the bundled
greylisting software (spamd(8)). There are other packages out there
that may be more flexible, and we also found cases where greylisting
does not work well, but in general, this is a good solution for us.

The cases where it does not work typically involves either the foreign
mail server not understanding 421 and bouncing the message (eg. Tobit),
or large mail clusters which try to send the message through ever
different members of their cluster, so they'll not get a whitelisting
before their queue hold time is up. I've seen at least one instance of
this problem with a British company (can't remember the name), who was
literally sending from a whole class C network.

If someone has an idea about how, and why, such mail clusters don't
send one message through one IP, I'd like to hear the explanation.

You typically only find out when mail does not arrive...
