a quick peek at Q-S code reveals you get this either by:
- having no MIME-Version header (but one gets added anyway if it's missing)
or
- having CR or NULL in some header(s) (the pattern match is /\r|\0/)

only you can look at the raw, quarantined email. 
try 'cat -vE filename' and look for "bad things"
   CR will appear as "^M" for example
