Sorry for the list flooding.. but the situation has changed. Following a
pretty much full reinstall to try and fix this problem, I'm reallly tearing
my hair out.
Now what happens is..
./test-installation -doit
works as expected.
All remote email just dissapears.. no logs, no bounces, no nothing.  In
over an hour of waiting nothing happens.
As soon as I edit /etc/tcp.smtp and remove the QMAILQUEUE line then mail
goes through magically.

For those that missed it,

Perl:    Summary of my perl5 (revision 5.0 version 6 subversion 1)
configuration:

Scanners: perlscanner, clamscan_scanner, spamassassin

Scanner versioning: clamscan: 0.24. spamassassin: 2.41.


Everything is pretty much default except for clamscan which was recompiled
using the fix on the list involving reducing that DEFINE, and softlimits
have been raised to over 50000000 in tests just ot ensure they aren't the
problem.
