If someone makes a SMTP connection, and waits until it times out, qmail-queue/qmail-scanner is still called by qmail-smtpd. This is a Qmail question - not Q-S really. Again, it's when a SMTP session is killed for whatever reason half way through. e.g. the SMTP client sends 99% of the message, and then the (client) server dies due to a disk failure. You end up with half-written files on the (Q-S) server.
