Turn on DEBUG in the qmail-scanner-queue.pl file 

#Want debugging? Enable this and read $scandir/qmail-queue.log
my $DEBUG='1';

Also make sure that you have the debug log file set.

#Name of file in $scandir where debugging output goes
my $debuglog="qmail-queue.log";


This will show you each step of the qmail-scanner process as it
executes.  It may be as simple as a problem with one of your virus
scanners.
