Have you checked your Qmail logs to see what happened to that message? You say Q-S is working fine - so the problem occurs afterwards?
I see you are using Thunderbird - so you *ARE* using an antispam system - are you sure you haven't configured SA to do things with Spam? Thunderbird defaults to Bayes tagging mail - but you can filter on it - have you done so? And if you are referring to Outlook 2003 - it also includes antispam support - are you sure this mail didn't end up in the Junk folder?
