The configure command I used to install qmail-scanner is as follows:

./configure --admin postmaster --domain mydomain.com --scanners
clamscan,verbose_spamassassin --notify all --add-dscr-hdrs yes
--ignore-eol-check --skip-setuid-test --local-domains "mydomain.com,
mydomain2.com" --install

(with mydomains replaced!)

I'd like to notify the recipient and admin, what can I check / try to
resolve this?
So you actually want:

--notify "recips,admin"

- even though you are strongly recommended NOT to do that? And you may
very well find yourself put onto RBLs due to your mail server SPAMMING
people with "a virus was blocked..." mail messages - when they weren't
actually involved in the transaction?

[Can you feel the strong hint not to do what you are trying to do? ;-)]

Anyway, assuming you know better, you say the "recips,admin" option
doesn't work? I just reconfigured with that option, and two email alerts
were sent... Looks fine to me.

OK so I've edited qmail-scanner-queue.pl directly as follows and it works, so I'm not sure what was wrong with my previous configure command.

<snip>
#Addresses that should be alerted of any quarantined Email
my $NOTIFY_ADDRS='admin,recips';
<end snip>

Regarding your spamming point. I really dont want to SPAM anyone (get enough of it on my server!) hence partly why I wanted to turn off sender notifications. Should I also turn off recipient notifications? Even though the notifications should only be send to local users?

Thanks
Gary
