do you have a root alias? /var/qmail/alias/.qmail-root?
Do you have delivery instructions for root?
cat /var/qmail/alias/.qmail-root
where does it go? local, to another address?
If local, does the user have a mailbox set up right? if the .qmail-default
for that user is ./Maildir/
is Maildir available in the directory, and was it set up correctly?

How can I create a mailbox for root?
