Yes.  After you un-install everything (really, you should only need to
uninstall the qmail rpm, and probably fastforward I believe), you'll
have to do a rm -rf /var/qmail.  There could be messages in the queue,
etc.... things that are preventing rpm from removing the directory (if
there's anything in there not owned by the qmail rpm, it will not
remove those files or the directories).

I would just try removing the qmail and fastforward (you do need to
remove fastforward first if you want qmail to remove the /var/qmail
directory as fastforward does install some aliases and manpages and
such in /var/qmail).

I would just remove and re-install those two packages (on install, do
qmail first and then fastforward).  I'd also run
/etc/cron.hourly/qmail immediately after the install to rebuild the
user database and also force a cdb rebuild by using:

/etc/rc.d/init.d/qmail cdb

That should help you out.  Again, I can't see the problem here other
than it being a permission thing; but I've never personally seen or
heard of this happening with my rpms before.
