connection died error

I m facing a starange problem , when ever i m sending mail to remote server my 
mail server is giving below error in log file

1000227014.026817 delivery 132560: deferral: Connected_to_172.28.1.1_but_connect
ion_died._Possible_duplicate!_(#4.4.2)/

While remote server config is correct and i don't think so there is any issue 
related with b/w .
Can any one pl help me out !!
