We are guessing far to much.
The original poster should post:
-Operating System, version, patches applied
-output of qmail-showctl, _unedited_.
-URL to his complete qmail-send logs.
-first two columns of /etc/password (that is numerical uid and username).
Otherwise we can guess as guess can without finding the problem.
