Can someone tell me whether or not the new mysql valias stuff in qmailadmin 1.2.1 is going to harm a vpopmail/ezmlm-idx setup?

The .qmail-list files created/modified/used by ezmlm-idx are not stored in the valias table. This means that you can't have a chkusr patch for qmail that simply checks a few MySQL tables to see if a user exists.

The dotqmail2valias program in vpopmail will convert alias, forward and autoresponder .qmail files into valias table entries, but leave the ezmlm files behind.
