[qmailadmin] Add User page blank

I just setup a new qmail box with vpopmail 5.4 and qmailadmin 1.2.  Qmail is working fine and all command line vpopmail commands are working.  I can login in to qmailadmin, but when I click on add email account the page comes back blank.  If I create email accounts with vadduser they do not show up in qmailadmin.  I using mysql no cdb.   I do get this in my apache error log -> vmysql: sql error[5]: Commands out of sync;  You can't run this command now which I tried a google on and did not find anything helpful.   Jay
