I would begin by upgrading your toaster. It's really quite simple if you use the qtp-newmodel command. You'll find that included as part of the qmailtoaster-plus package (qtp.qmailtoaster.com). The wiki (wiki.qmailtoaster.com) contains instructions.
Before doing the upgrade, it'd be a good idea to configure your yum to access the rpmforge repository. There are some perl modules there which the newest spamassassin uses. You probably should consider upgrading your OS too if it's been that long. I wouldn't call necessary, but you probably should at least apply whatever security updates are available.
