I have already reviewed the archive regarding this error.  The most
pertinent and informative thread does not address, IMHO, the issue my
customer is seeing:

I have isolated the occurrence in the logs:

I have checked with the user that there were no CC/BCC recipients at the
same domain.   

 

Please let me know what information would be needed to assist in
troubleshooting this issue. 

I have already spent some time on Google.com and reviewing various qmail
references.  Any help is appreciated.
