I notice that you're missing ,NOP0FCHECK="1" at the end of each line in your tcp.smtp file. I don't know how/why that could be the cause of your problem, but I'd add it, and don't forget to run qmailctl cdb after it's changed.
What toaster versions are you running? Are you running spamdyke?
