I've just one thing to add to this. If you're running spamdyke, it's
fairly normal to see TIMEOUT messages in the log. These are spam
connections that don't go away gracefully, and they follow a DENIED
message of some sort. OTOH, if you see TIMEOUT message with no prior
DENIED message, that would probably result in a duplicate being sent
again later.
BL, spamdyke's TIMEOUT log messages are in most instances considered
normal. There can be exceptions though.

What I've seen is, when you server gets hammered, Spamassassin bogs down and
takes too long to process the message. This causes the sending server to
resend the message after timing out. You could also look through your logs
for "TIMEOUT", I believe.
If you're not running SpamDyke, do. It pretty much kills the issue with
Spamassassin getting overloaded, and eliminates, at least this cause for
duplicates.

I have suspected - but not proven that it *can* happen when the
following is true
the user get email from 2 locations [ dsktop & laptop ] at one of them
they are leaving the email on the server fro 5 days.
at the other it is deleted from the server upon reading.
for whatever reason this results in ... the flag not being set -- or
whatever and the mail can come down ...more than once.
