I have double checked and the user AND domain are both in the QControl
admin utility and the domain is in the /var/qmail/control/virtualdomains
file.
Where else should I look to debug this? This happened to email address
in the domain (same inbound email).

Is the domain in /var/qmail/control/rcpthosts ?
If so, is the domain in /var/qmail/control/users/assign ?
