Checking chkuser entries in the toaster's smtp log
(/var/log/qmail/smtp/current), I see that chkuser created the
reject log
entry at 4:02am.
The mailbox definitely exists and is used regularly.

I don't know for sure. I would expect qmail to return a 4xx
(temporary) failure message if spamassassin (via simscan) wasn't
available. At least that's what *should* happen.
The times do appear to be more than coincidental though. Can you run a
test? Shut down spamd, send a message to QMT, and see what happens.
To test, I changed firewall rules to block inbound from the public, then
as root ran these commands copied from qtp-sa-update:

chkuser would use vpopmail to check the database for the existence of
the account. What was the status of mysqld when the error message was
issued?
I looked at /var/log/mysqld.log. It doesn't have a lot of detail, looks
like only messages related to mysql stopping or starting, which happens
pretty infrequently on this machine, looks like only when it gets rebooted.
I looked in /var/log/messages and there was no mention of mysql.
Any idea where else to look? Thanks for your pointers, Eric.
Looking back in my logs, looks like this is the only time this has
happened in the last month.
