Observed when running .Net tests, but I think this is a broker problem. Setting
a prefetch size of 1 (or even 2 or 5...) on a BasicQos, stalls the message
flow. Generally, 1 or 2 messages will get through on the channel, but the flow
is suspended after that and does not resume, even though acks are being sent
back.
