I had the same problem once and found out that the lookup for QML plugins is 
case sensitive.
So if your QTDIR really is C:/qt/qt5 (or some other inconsistences between 
configuration and "real" path names) the plugin loader does not find the dll.
I did not file a bug report, although I should have.
