Check the Medicare Home Page at: http://www.medicare.gov/ 
 
Click on "Formulary Finder" on the first Page.  This tool will allow you to find plans in your state that match your required drug list.  There are Plans for different states so you can find a Plan for AZ by looking under the "Compare Medicare Prescription Drug Plans."
