Obviously this works best with empty bowels and bladder, and it helps if you don't take it after a heavy meal. They say that you do need manual assistance, but about 1 hour after I take it I am at attention and it stays that way. I don't know if it's mental or what but after the act is over it goes away (until the next morning) ...round 2!
