It's true that many programs included the OS, but a lot did not. Including it was generally more of a convenience than a necessity. Once you booted into Prodos 8, you could easily switch betwwen programs without needing the boot disk so it wasn't necessary to have it on each program disk. GSOS (System 5 and 6) was similar to the Mac OS and various files were accessed as needed. Not everyone needed the same bits and pieces so most GS users configured the system disk to their personal requirements.
