Rethat 7.2 (I may be off by a few tenths) and probably others of that time shipped with the IDE driver defaulting to using PIO rather than DMA. It was pretty easy to miss interrupts if you had a lot of disk activity. (I had a test program that would drop zillions of them.) -- These are my opinions, not necessarily my employer's.
