Writing R Extensions is an *complete* and *up to date* documentation for this task. Package management is being steadily improved.
Really, I recommend to use "Writing R Extensions"!

If you are working on Windows, you might want to look into the R Administration and Installation manual as well which descibes how to collect and set up the required tools...
