Yes, that's a reasonable compromise. If I want to move my code to
another implementation, I can put #!case-fold at the top and that
issue will be resolved.  People coming from case-sensitive
implementations can add #!no-case-fold to the top of their code and
win, too.

But what happens at module boundaries?
