Yes this is possible, simply specify the AcctColumnDef twice.

Ie:

.....
AcctColumnDef TERMINATECAUSE, Ascend-Disconnect-Cause
AcctColumnDef TERMINATECAUSE, Acct-Terminate-Cause
.....
