When I went over last year I rented a SIM card through JCR Corp:

Website's nothing flash, and prices aren't cheap, but that's the case across 
the board from what I've found. They delivered it to my hotel the day I arrived 
in Japan, with an envelope to post it back to them once you're done. The SIM 
worked fine, they give you the details to get data working, and Docomo (the 
carrier they use) has a pretty solid network.

For those with iPhone 4 and iPads, they should offer MicroSIMs (they didn't 
officially last year, but Ben Hoskings asked via email and they were happy to 
supply one).
