Frontpage will generally only interact with Microsoft's Internet Information 
Services (IIS), which is the bloated and hard to configure alternative 
webserver. I think to run the form, you need to install the "FrontPage server 
extensions" which will probably not run on Apache.

I think your only options are to convert the HTML code to something that Is not 
Frontpage specific (ie, no code that calls MS server functions, so it is not calling 
things that are not available to Apache) or install IIS, and Frontpage Extensions, 
instead of using Apache.
