   I expect Kenny stole the idea from a very old joke told in Sales meetings 
when I was in my 20's... Goes like this:

   On a Sunday when the local Preacher was heading into the home chute of a 
fiery sermon, he said to his parishioners, "By a Show of Hands, How Many Of You 
Want To Go To Heaven?" .

Up shot the hands of every parishioner, save One somber looking fellow in an 
aisle seat near the back of the Church...

The Preacher said "What's the problem Brother ?  Don't You Want To Go To 
Heaven? Surely you Must!!"

The man replied succinctly "Sure I do Pastor, But It Sounded Like You Were 
Trying to Get A Load Up To Go Today!"

That being the likely root of the Axiom "Everybody wants to go to Heaven, but 
No One wants to Die"
