Steve,

Make sure that the initial installation was successfully completed
on the "same" workstation/server prior to installing any on-going
updates, etc., keeping all user permissions in mind.

Not to mention the exact version of R:BASE 9.1 vs 9.5 and 32 vs 64.

Hope that provides you with some blue's clues ...
