That exact behavior is the result of multiple rows associated with the
Source Table in the Variable Lookup ListView having the exact same return
value.

While in form designer, you will need to revisit the ListView and make
sure that the returning "Variable" is assigned to a unique "Key Field"
column in the Source Table/View.
