Thanks to Ben, Troy, Jim, and everyone else who responded to this question.

What finally worked was to reboot the server and then to reload the
database.  I checked it today, 10 days after the reload, and there were no
ghost users.  I have no explanation whatsover for how this is possible.

Note: This is under Netware 4.11 SP7
