I use AuthorIT. IT costs $150 US and create full indexes (in the Windows
Help alternative with absolutely no extra input from me. It has the
ability to create HTML help as an alternative to Windows help but I have
not yet tried it to see if it creates the same full text indices as the
Windows help.
There is a trial version downloadable from www.author-it.com.
