Sami, Howz about something like this (very rough):
Set up a table for your boiler-plate, choosing some character to identify where you want data filled in by the user, and have a second column with text (a portion of the boiler-plate) that would identify the end of the boiler plate. Your note field would be a variable, and on entry an eep would evaluate the variable. If null, it would start with the first segment of either a predetermined phrase, or present a choose menu to start. The eep would parse the the boiler-plate to the first insertion point and set the note variable. The user presses enter, and on entry, the eep would evaluate the variable to see if the variable contained the ending text to the boiler-plate. If not, it would add the next text segment and skip back, append the next segment and playback the '[End]' key , and repeat the cycle until the final text segment was added. Kind of fussy, but a thought.
