Please don't take this as insultingly simple, but are you sure you are
getting the QR tight enough? I've used QRs on all my many fixeds with
absolutely no problems, **as long as** the QR is a sturdy, all steel
internal cam type, and as long as it is very tight.

You also might look to see if the axle extends even very slightly beyond the
outer face of the dropouts.

Practice. I generally have the QR lever on the left (not always, if I flip
the wheel), and then I simply reach out with my right hand and grab the
front edge of the tire and at once pull it back and center it. While I do
this I tighten the QR with my left hand.

If your chainring is not centered, as many are not, then getting the right
chain tension will be more difficult.
