I've only had one chain fail on me.  Attributed it to my sloth in 
monitoring.  One of the tendancies I've noted is that on a simple 
drivetrain (fixed/SS) you tend to just blissfully humm along without worry 
- everything always works.  Until it doesn't.  On the chain that failed

I've typically used 8 speed, but started using a chain tool after the 
failure. 

There's a lot more chain stress on a derailleury bike, of course - at least 
laterally.  I tried a 1/8" chain on my SS mtb and didn't like it - too much 
noise and lateral movement on the sprockets and I didn't want to change 
everything to the wider stuff.
