Hi Kevin,
I had a similar occurrence with a drive side arm but my fix was as simple as 
cleaning off the spindle, re-greasing, and reinstalling and tightening a little 
tighter than before.  
This was a NOS ritchey made by Sugino.  After the above steps my problem was 
solved although I did remove these cranks to save for another project anyway 
but the loosening didn’t occur again during that time.
Without wishing to start a grease vs anti-seize debate I’ll just ask if you 
used ANYTHING to lightly lube the tapers?  My understanding is that something 
(whatever’s your cup of tea) is better than nothing and helps with a nice tight 
fit just like with a greased bolt snugs up better.
