I installed 2006R4 and could double click on projects to run the IDE.
I've not been able to change the associations to the newer releases of
realbasic as I installed them, so double clicking on projects is broken for
me on the Mac.
I'm sure this is my issue of not being familiar enough with the Mac to
tinker under the hood, but it is annoying.

Ditto - Double-clicking projects has always worked for us also -
never broke and never had to use "open".
