It's certainly possible to create virtual ports on-the-fly on Mac OS  
X, but (AFAIK) the only way to create ports on Windows is with a  
driver of some sort, like this one:

Other than that, it's not too difficult to manage MIDI with declares.  
I have a Windows-only example available if you're interested:

It's a little rough around the edges - OK, a lot rough ;) - but it  
does the job. I haven't got around to adding Core MIDI support (yet)  
or sysex however.
