I used the tutorial at http://opensimulator.org/wiki/ModRex when I did mine. It seemed to work for me. The only thing I noticed was that to run simxtd, I had to put ./ in front of it. Everything else seemed to run correctly.
Its just that afterwards, meshes weren't showing up.
