Well doing a bit more research, you add them on the server side, (another
problem I let SimHost know :)
But Im not real clear if you model a clothing item? Lets say you want to
make a ladies skirt. Do you have to load up the detail avatar in your
modeling program, model the a skirt shape, add materials then export that
into the right server folders as a separate object? or are the 'clothes'
just templated (UV) material files like the clothing in SL?
Perhaps the skirt is a 'attachment?'

At least you can change the outfit in the avatar generator, for example
switch a
shirt. It is not modelled on the avatar in the sense that it'd be a part
of the
same mesh, but another mesh which is attached on top of it. It seems that
the av
code can morph the clothes so than when you e.g. change the body mass or
muscle
size of the av, the clothes resize automatically too.
I don't know yet myself how exactly it goes when you add new clothes,
where they
become available and what does that take etc. - hopefully someone or docs
on the
web tell us.
