You'd be surprised how often the dumbest "can't happens" happen!
Glad you found it.  I was going to suggest that
the other thing that can get you is entries in /etc/hosts.deny and 
hosts.allow, which telnet (and other xinetd controlled services, as
well as some other services) pay attention to.
